The Best Community Program celebrates mainstream community-based organisations whose programs showcase inclusive practices. NDIS Providers or disability organisations are eligible to nominate only if they are supporting the mainstream community organisation to establish their program.

Nominee Details

This is the name and details of the person being nominated for the award.

Award Nomination Criteria

1. Provide the name of the program, who are the key stakeholders and an outline of the program objectives. (maximum 1000 characters).

2. Tell us about program, with a focus on the activity type, the program’s frequency and the participation numbers (especially show us the growth of the program). (maximum 1000 characters).

3 What are some of the outcomes achieved for individuals who are involved in the program. The inclusion of a Participant testimonial would be advantageous. (maximum 1000 characters).

4. What changes have you seen in both the Participants and the organisation’s community and what is the impact of this? (maximum 1000 characters).

Referee Details (Optional)

You can provide the details of a referee to support the nomination and the statements that are included in the nomination.

Local Media Contact

Help us promote and recognise the nominee through local media channels by providing at least one local media contact.
